Living Towards Death: Patient-Centered Care at the End of Life

Featuring: April Price, RN, CHPN, Regional Director of Clinical Specialty – Hospice

Dying patients may experience feelings of grief and loss in many forms and with varying effects. As healthcare providers, it can be difficult to approach the subject of death or have confidence providing advice and guidance through this difficult time. As a result, seeking our counsel may not benefit from the specialized care that partnerships can provide.

Confidently understanding and assessing each individual’s medical, spiritual, and psychosocial circumstances, while focusing on the patient’s personal preferences and goals, will help guide in supporting those living toward death.

Following this presentation the participant will be able to:

  • Confidently initiate advance planning and end-of-life conversations
  • Possess the ability to design a patient-centered plan of care or treatment
  • Proficiently differentiate and identify when care transitions from palliative to curative
  • Possess a working knowledge of the functions and benefits of healthcare partners
