We’re looking for a highly capable, purpose-driven team member who is ready to take ownership of this role and run with it—building deep community relationships, driving growth, and creating lasting impact for older adults and their families.

This is not a “check-the-box” sales role. This is a builder role for someone who sees opportunity, takes initiative, and is energized by meaningful work. In this role your time will be focused on developing relationships with professionals such as home health agencies, skilled nursing facilities, hospitals, discharge planners, social workers, and other healthcare providerswho regularly support older adults and their families.

This position offers a competitive base salary of $60,000–$70,000, depending on experience, along with performance-based bonus opportunities of $15,000–$20,000 annually tied to referral network growth and partnership development.

Core Responsibilities

  • Build, deepen, and sustain relationships with hospitals, physicians, assisted living communities, elder law attorneys, case managers, hospice partners, rehab centers, and more
  • Proactively identify new referral opportunities and nurture existing partnerships
  • Plan and execute intentional outreach, 1:1 meetings, and group presentations
  • Track relationship activity, progress, and outcomes in Salesforce
  • Collaborate with leadership on weekly and monthly growth strategies

What You Bring
